It's a bus you drive in the desert. Also the driver is dead because he's tired of driving the bus for 8 hours, it's one of the 6 games from the unreleased Sega CD game "Penn & Teller's Smoke and Mirrors". It's got quite a legacy here, not bad for one of the worst video games ever made.
